A NEW CHURCH DIRECTORY IS PLANNED FOR THE END OF SEPTEMBER.

 

Although we are constantly in the process of updating our member information as we know of changes, we really need your help.

Please take a few minutes to make sure we have your current correct contact information. We also would like to include your photo in the directory, which you can attach to the bottom of this form.

If you have anything new that should be added or deleted, please let the office know.
